By registering your child for this class, you give consent for your child to participate in class. You agree to assume all risk of your child participating in class. Without clicking agree, your child will not be able to participate in class. You acknowledge that the activity may pose some risk of personal injury and that you undertake and assume this risk for your child. All students must be covered by personal medical insurance. In case of illness or injury, you consent that the instructors have the right to administer first aid and/or seek medical services. The cost of any medical services received is the responsibility of the parent or guardian. The insurance for your child will be primary. I, my child, and all heirs and assigns, agree to release, waive, covenant not to sue, indemnify and hold harmless Nina Parrott, the Fifth Flavor, any other adult working in the kitchen with Nina, and any other agents of any and all liability from negligence and responsibility of any nature for any loss or damage to property or personal injury, including death or illness, incurred by my child while participating in the activity.
